Contract Bonds > Contracts over 500K

Bonds in this range require the following information:

1. Contractor Questionnaire (646kb), complete all questions on the initial submission (include work & supplier reference letters).

2. Business financial statements, furnish a copy of the last three Fiscal or Calendar Year-End Financial Statements on the company, if available. In addition a current Interim Financial Statement if the Year-End is older than 90 days. All Financial Statements are to be prepared by a CPA or Public Accountant in accordance with AICPA Guidelines established for construction accounting. They must include, as a minimum, the Accountant’s Opinion Letter, Balance Sheet, Profit & Loss Report, and Statement of Changes in Retained Earnings/Owner’s Equity and Footnotes.

   NOTE: All Financial Statements must have the owner’s or Officer’s signature with a date on the Accountant’s Opinion Letter or on the balance sheet.

3. Personal financial statement (214kb), personal financial statements are required of the principal owners (20% or more ownership) of the company. They must be signed by both the principal and spouse.

4. Upon approval, we’ll prepare a General Indemnity Agreement for execution by the principals, and their spouses.

5. Resume (165kb), of all principals of the company are required.

6. Bank reference letter (216kb), complete the upper portion, sign at the bottom and have your bank complete the remainder.

7. Bond request form (195kb), complete for a bid or final bond. Please complete all sections and send bond form & contract.

8. Current work on hand (255kb), this form must be within thirty (30) days current at time of submission.

If credit and project references return satisfactory, we can demonstrate experience in the type of work to be performed and reflect adequate capital to finace the work, a bond will be provided.
